For consistency and ease of use, please follow the guidelines below for entering data on this form:

1. Enter all surnames, including that of the member submitting the surnames, in upper case (capital) letters.
2. Enter only one surname on each surname line. If you are entering alternate spellings of the same name, please enter each on a separate line so that they will appear in alphabetical order in the database.
3. There is space available on this form to submit ten surnames at once. You may submit more than ten surnames by filling out and submitting the form as many times as needed. There is no limit to the total number of surnames a member may submit.
4. Use the tab key or the mouse to move between the fields on this form. Do NOT use the enter key. The enter key will cause the form to be submitted.

The remainder of this form is for entering each surname you wish to submit and the location(s) for that surname. Please enter surnames in all capital letters. And please use the 2-digit state abbreviations in order to fit more locations on one line. For example, if you are researching the surname Waring in Indiana, Kentucky, Maryland, Virginia and England, enter the surname as WARING and the location(s) as IN, KY, MD, VA, England. If you wish instead to show the direction of migration, enter the locations as: IN<KY<MD<VA<England, or if you prefer, England>VA>MD>KY>IN.
