Georgia did not require Death Certificates until after 1900. The following is a blank page from the Death Record Book that the physician would go by the courthouse and fill out. This was scanned from an old copy of an old book (ca 1930's). What the procedure was from there to send to the State, I am not sure. Also, from the scarity of death certificates before 1940 or so, either many people did not call a physician if the person was already dead, or the necessity of recording had not been really impressed at this point in time. On old certificates many lines are blank or marked unknown.
