The chart below is made for a family of 12 children. 
It is only 2 generations since there is no simple way to do more than that and store it. 
To simplify filing these and prevent duplication, we will only be filing the chart under the *father's* surname.
We hope that you will also submit a separate chart on the mother's family (mom, dad & siblings). If there is more than one marriage, list the 1st mother above her children, leave a line after her children's names, then list the 2nd mother followed by her children.
