Instructions for Submitting Your Family Group Sheet
The chart below is made for a family of 15 children. To simplify filing these and prevent duplication, we will only be filing the chart under the *father's* surname. We hope that you will also submit a separate chart on the mother's family (mom, dad & siblings). If there is more than one marriage, then please do another group sheet & number it as #2 by husband's name.
DO NOT include LIVING PERSONS on these charts (due to privacy concerns, they will not be added)
Put all SURNAMES in capital letters
List all women under their maiden names, if unknown, then, please list as such
- If you are making corrections or additions to existing Families you have already submitted, enter the Husband's name (or wife if no husband) and the. Leave all other information blank. Be sure to enter your name and email address.
- Add any additional information in the area left for notes, below the children.
You are strongly encouraged to quote your sources on the bottom!!! An example on how to create a well documented FGS: http://www.rootsweb.com/~usfgs/newmexico/sources.htm
- NOTE: After submitting your entry, you will be taken to a screen titled MailMerge Gateway. Use your browser's BACK button to get out of this screen and to continue sending in other family group sheets.
DO NOT push the "Submit" button more than once for each group sheet.

showing your group sheet submission. Your group sheet will automatically be mailed to me, the GA
Family Group Sheet Project
& one will come to you. You may send this one to another
state or county that might be interested in this information for
use on their web sites.
