To The United Daughters Of The Confederacy.
The undersigned, residing at Upson Co. Ga. Who is an
ex-Confederate soldier, and a member of Camp No.
1480 U.C. Vs., hereby, at your request, presents
this Certificate of Eligibility for a Confederate
Cross of Honor. He entered the service of the
Confederate States on the __ day of April 1864, as a
Member in Company A of the 5th Ga. Reserves
Regiment of the GA. Volunteers For The War C.S.A.
and was at that time a resident of BIBB County, Ga.
He was honorably discharged from service by
SURRENDER on the 26, Day of April 1865, at which
time he held rank of PRIVATE.
