To all whom it may concern
Know ye, that John M. Collett, a private
of Lt. Francis Otwell's Company "94",2nd Bat. of Vetren Reserve Corps Volunteers, who was enrolled on the
twenty fifth day of August one thousand eight hundred and sixty two to serve three
years or during war is hereby discharged from the service of the United States
this thirtieth day of June 1865, at Indianapolis, Indiana by reason General
Order No. 116, War Dept. A.G.O. No objection to his being re-enlisted is known
to exist*, Said John M. Collett was born in Warren
County in the state of Ohio, is twenty one years of age, 5 feet 11 1/2 inches
high, Light complexion, Black eyes, Dark
hair, and by occupation when enrolled, a Laborer.
Given at Indianapolis this
thirtieth day of June 1865.
*This sentence will be erased should there}
be anything in the conduct or physical
} Commanding the Regiment
condition of the soldier rending him }
unfit for the Army.
