Note:  an application and photo identification are required (a photocopy of a driver's license, work identification card, etc.).  The fee for a genealogy hard copy is $4.00 and a certified copy can be obtained if relationship can be proven for $5.00 for birth or death certificate.  If the exact date of birth or death is not known, please provide an approximate 5 year period.  An application and instruction sheet can be requested by e-mailing jayvitalrecords@hotmail.com with the request.  If there are any other questions please call the Jay County Health Department at 260-726-8080.
