a.   Application Statement:  State of Ky. County of Clay, 15 Sept. 1887
 appeared before county clerk -  States that he enrolled on 11 Aug. 1863 
in Co. D. 47 KY Mounted Infantry commanded by Capt. D. M. Clark.  Was discharged
 at Lexington, KY on 26 Dec. 1864. 

Personal Description:  age- 51, Height -6 ft., complexion- light, eyes - blue .  

While on duty at Camp Wilson, Estill Co., KY he contacted measles and from 
exposure he took cold on them; resulting in lung disease.  At Paris KY in 
Aug 1864 he contracted piles and kidney disease by exposure.  He was 
treated at the hospital at Camp Wilson. Since leaving the service has
 resided in Clay County in the occupation of a farmer.  He says he is 3/4 disabled.

b.  Affidavit of Origin of Disability:
Before Joseph Allen JP. David C. Cole also of Co. D 47.  States that on
 Sept. 1863 at Camp Wilson David Bishop was taken sick with measles and
 was sent to the hospittle (sp) and i think taken cold and same caused 
lung disease.  At Paris he got piles and I saw bloody discharges and I 
waited on him myself.  He seemed to suffer.  His urinary organs seemed
 to pain him and he had dark urine.  
(Coles address was Tankerley, Clay.  Called himself a comrade.)
 

c.  War Dept. Affidavit:  Wash. D.C.  Attny Gen. Office Jan. 26 1888. 
 (David) on muster roll Aug. 1863 at Irvine; left sick at Irvine KY
 Oct 1863 rejoined Feb. 28 1864.  Army records of unit not on file. 
 R.C. Drum.  Southern Division Claim # 624339.
Benj L. Allen was listed as Examiner of Clay Circuit Court.

 d.  Affidavit by Granville Combs aged 45 of same army unit. 
 He first saw David after his discharge at his home on 1 Jan 1865
 that D was in bad health etc lungs back kidneys.  Had not lived more 
than 4 miles apart since war.   

 e.  Affidavit of Private Joseph Allen:  Same facts.

f.    Statement by David Bishop:  Nov. 16, 1887 Claim 264339.  
He said Dr. Daniels of the Regiment treated him but is now dead.

 g.   Statement by David Bishop Jan 26 1888:  Teges as residence ever
 since discharge:  Dr. Glass of the regiment treated him.  Dr. M.V. Morgan 
of Sextons Creek Clay  has doctored me some after war.   

h.  Statement by David Bishop  Mar. 15, 1888:  Dr. M.V. Morgan had treated
 him years before but now could not recall. (memory impaired.)

 i.   Doctor’s  Exam:  (Can’t read name)  PO of the Board: London Ky.
  PO of Claimant;  Tedious Ky.   Certifies to his illnesses.  Pulse rate
 65/ min.,  respiration 22, temp 98.5,  height 6 ft.  weight 150 lb.. age 51.   
He is pale and emaciated,  bronchitis. 1/3 disabled.

 j.   Death:  May 28, 1889.
