Declaratoin for original invalid pension
State of Kentucky
County of Clay
On this day of March, A.D.1888 personally appeared before me, a
clerk of the
county, a court record within and for the county and State
aforesaid, Jesse
Barrett, aged 53 years, a resident of the Creek of Tedian (Teges)
county of
Clay, State of Kentucky, who being duly sworn according to the
law, declares
that he is the identical Jesse Barrett, who was enrolled on the
19 day of
August, 1861, in company B of the 7th regiment of Ky. volunteers
commanded by
Capt. Daniel G. Williams, and was honorably discharged at
Louisville, Ky. on
the 5 day of October, 1864; that his personal description is as
follows: Age,
53 years; height 5 feet nine inches; complexion, swarthy; hair,
black; eyes,
yellow. That while a member of the organization aforesaid, in the
service and
in the line of his duty at Mullicans Bend, in the state of
Louisianna on or
about the 27th of March, 1863, he was taken first with diarhea
and fever and
which ? ? of liver and kidneys and disease of lungs, have a pain
sto!
mach and swelling of same and a pain in the small of my back ?
and ? have a
bad cough with a ? ? and ? phelgm and an inability to lay on
either side and
a general debility being very weak. That he was treated in
hospitals as
follows: first at ? hospital ?? 15 April 1863 and from there
Jefferson
Barrack ? think sometime in May 1863.
That he has not been employed in the military or naval service
otherwise than
stated as above.
That since leaving the service the applicant has resided at
Tedians of Clay
in the State of Kentucky, and that his occupation has been that
of farmer.
That prior to his entry into the service above named was a man of
good,
sound, physical health, being when enrolled a healthy man. That
he is now
three-fourths disabled from obtaining his subsistence by manual
labor by
reason of his injuries, above described, recieved in the service
of the
United States; and he therefore makes this declaration for the
purpose of
being placed on the invalid pension-roll of the United States.
He hereby appoints himself as his true and lawful attorney to
prosecute his
claim. That he has not received no applied for a pension. That
his post
office address is Tedians, county of Clay State of Kentucky.

Also personally appeared John S. Herd, residing at Manchester, Ky
and George
W. Baker, resideing at Laurel Creek, Ky. persons whom I certify
to be
respectable and entitled to credit, and who, being by me duly
sworn, say they
were present and saw Jesse Barrett, the claimant, sign his name
(or make his
mark) to the forgoing declaration; that they have every reason to
believe,
from the appearance fo said claimant and their acquaintance with
him, that he
is the identical person he represents himself to be; and that
they have no
interest in the prosecution of this claim..
