Membership Application Forms

2. Complete the application, print it on legal-size (standard application) or letter-size (short form application) acid-free paper, and mail it to:

When using the standard application, include documentation supporting descent from the eligible ancestor. When using the short form application, include documentation supporting the claimed lineage from the applicant to the member relative.

A non-refundable $25 processing fee must also be included. Upon approval, there are four membership options:

• A one-time payment of $250 for Lifetime Membership.
• A $20 payment each year for Annual Membership.
• A one-time payment of $1,000 for Patron Membership.
• A one-time payment of $5,000 for Benefactor Membership.
