Submissions not following these simple guidelines will be deleted!

1. Please do not post death certificate data for people who where not connected to Barry County. If they died in Barry County or were buried in Barry County then you can submit the death certificate information here.

2. Please post only what the certificate reads. Do not add your own information or what you think that the certificate should read.

3. The note area can be used to add other things that might be written on the document or the supplement to the document.

4. If you know the information given on the document is in error then add a note at the bottom that explains what is incorrect - but be sure and add all of the information the document has written on it. Use the boxes for all that information.

5. Babies and males don't have maiden names! Use this for females who are married. Example: Mary (Smith) Jones. In this case Smith is the maiden name.

6. Use standard English and standard abbreviations. Death Certificates will have the old version of the state abbreviations because of the time period that they were filed. Please use the modern day usage, because we are going to chart these on web pages. See examples below for more information.

7. I will not accept entries that I can not understand. Retyping information you submit creates a lot of unnecessary work. Be accurate and do not change the information that is written on the document.

8. The coordinator reserves the right to reject or edit any or all entries that you submit.

Such as MO - not Mo. 

Correct usage example: Seligman, Barry Co., MO

Such as Barry Co., MO - not Barry co, Mo. - not Barry Co, MO

Correct usage example: Barry Co., MO

Such as TN - not Tenn.

Correct usage example: Greene Co., TN

Such as Co. for county - not Wayne Cty Ky. 

Correct usage example: Gap Creek, Wayne Co., KY

Such as Flat Creek Twp. - not twsp or TWP.

Correct usage example: Flat Creek Twp., Barry Co., MO

For Dates: Use this format Jan. 1, 1940 - not this one - 1 Jan. 1940.

If a female with additional married names, place the additional marriage information in the note section at the bottom of this form. Do not list unmarried females, males or children maiden names.

You may now enter additional information given such as the something the doctor may have reported or a place where the burial was if other than Barry County.

DO NOT ADD ANYTHING THAT IS NOT WRITTEN ON THE DOCUMENT. (Please be brief.)

DO NOT USE UPPER CASE WORDS FOR SURNAMES in the research note as text.

Example:

Not HADDOCK - use Haddock.

A note at the bottom can explain when facts seem to be incorrect. 

Example:

Note: Bob's stone reads that he was born in 1896 not 1886 as the death certificate reads. His age at death that is given on the death certificate would make the 1886 date the correct one.
