certify that I am of legal age and that I am personally
acquainted with Jane Feugate,  that she is a niece by marriage
and I have known her all her early life.

I further certify that I know to my best knowledge that she
was born on January 2, 1884 and that I have explained the
permanent Record of Births on file in the County Clerk's Office
which contains the record of her birth, but has ommitted the
given name of the above, I further certify that the Volumn No. 1
page 71, showing a female child born on January 2, 1884 is
the same person and should show the name of Jane Feugate.
