As of 1 Jan 2006, the fee for a State of NJ vital statistic record, such as birth, death or marriage certificates, will be $25.

When ordering the records, individuals must include identification and state his or her relationship to anyone else whose vital records are being requested

Acceptable forms of identification include:
A valid photo driver's license or a photo ID card.
A valid driver's license without a photo and an alternate form of identification with current address.
Two alternate forms of ID with current address.

Exact information must be included that identifies the name of the individual as it is recorded on the vital record, the type of event (birth, death or marriage), the date the event occurred and the name of the municipality in which it occurred.
