Whereas a church _____ had for the time of about thirty years existed here & has occupied as Union Methodist known for the purpose of religious meetings situated in the town of Coxsackie   County of Greene & State of New York & has been during that time occupied for that purpose by the different denominations of Christians, for Religious Meetings & the same has never been incorporated or its officers legally appointed to govern the same in its divine   that a corporation be created to govern & control the said _____ a Chapel & whereas proper notice has been given in pursuance of that statute for that purpose.

Now therefore as the undersigned, two of the members of the Congregation who steadily worship at ___ Meeting house or Chapel having then that day elected to preside by a majority of the Congregation at a meeting appointed & held in such Meeting House or Chapel for the purpose of ____ do herby certify that on the 4th day of October 1858 the male persons of full age belonging to ___ Society in which divine worship is Celebrated for the purpose of incorporating themselves & did then and there elect by a plurality of voices  Oliver Butler, William Webber, John Holister, Rulandus Smith & Lorenzo W. Hill as Trustees of said society aforementioned.

And the said persons said then & there also determine by the like plurality of voices that the Trustees & their successors affirm ____ shall hereafter be called & known by the name of ___ of the Trustees of the first Union Meeting House or Chapel in the town of Coxsackie.

Greene county  ss    On this 21st day of Dec. 1858 before me personally came William Webber subscribing witness to the above instrument to me known who being ___ duly ___ did  affirm & ____ that he resides in the town of Coxsackie & said County that he knew Enoch Lacy & John Holster the individuals subscriber in & who executed the foregoing instrument  there the was first & ____ the said Enoch Lacy & John Holstier sign seal & deliver the same as & for _____   _____  _____ & that the same Enoch Lacy  & John Holister acknowledge it execution thereof  whereupon it ___ William Webber became the subscribing witness thereto.
