We the Minister and Elders & Deacons of the Protestant Reformed Dutch Church of Leeds being duly assembled do Certify whomsoever it may concern that by lease of the proper ecclesiastical authority we have assumed the Title of the Protestant Reformed Dutch Church of Leeds & according to the Statute in such case made and provided with be known in law as a body Corporate by that Title In witness whereof we have hereunto set our hands & seals this twenty seventh day of April in the year of our Lord one thousand eight hundred fifty two.

County of Greene on this 29th day of April 1852 appeared before me James W. Elting whom I know & being duly sworn says he resides in the town of Catskill that he knows John Minor, James Van Deusen, John Van Vechten, Elisha Blackmar, Garritt W. Sager, Zerah Ferry, Benjamin Chidester & Isaac Plank to be the persons above described & who executed the above instrument that the saw them execute the same severally & that he subscribed his name thereto is a witness to such execution
