Certified copies of certificates may be obtained from two sources:
 Clermont County General Health District and the Ohio Health Department.

Uncertified copies for the years 1913 - 1944 are available for the Ohio Historical Society
for a fee of $7.00.   1909 - 1912 and 1945 - 1953 can be purchased by using this form.

Ohio death certificate images for the years  1908 - 1953 are free from
FamilySearch

This facility has a computer workstation where all certificates may
be viewed and is free for public use.   Requesting a paper copy requires
a fee of $16.50 and a certified copy will be issued.
Hours are Monday - Friday 8:30 AM - 4:30 PM

Mail in requests are $18.00 and may be requested using this form.  Include a SASE.
