  To whom it may concern, I Geo. W. Bishop, an undertaker of Stella, Ohio was called to the home of one Samuel Fort, a soldier who had died of complications of aliments in Benton Township, Hocking County, Ohio the 28th day of April 1901.
  I further affirm that I prepared the body of said Samuel Fort by embalming and furnished the bill of goods namely, Casket, Robe, Box, Hearse, and Attendance for the sum of $35., which was thus the governments allowance for the burial of indigent soldiers. I further state that I buried said Samuel Fort  in Wesley Chapel cemetery in Benton Township Hocking County, Ohio on April 29th 1901. I further state that the Reverend Wilson a now deceased Minister preached said funeral.
  To all this I solemnly affirm to be the whole truth  and nothing but the truth.
