Claim of John
Simonton No. 1108346
State of Ohio, Montgomery Co. On this 26th
day of April, 1892, personally appeared before me
a notary public within and for the County aforesaid
John S. Simonton
aged 56 yrs. a resident of the city
of Dayton, Montgomery Co., Ohio, who being duly sworn ac-
cording to law declares that he is the identical John
Smith Simonton who
was enrolled on the 25 day of
April 1861 as a private Co. “a” 12’ Reg;t. O.V.I.
That he
was honorably discharged on the 18th day of August,
1861 at Morrow, Ohio. That he is totally unable to earn
a support by manual labor by reason of being crippled,
caused from dislocation of right ankle, ankle broke
also, resulting from being thrown from a buggy attached
to a running horse on the 20, day of Oct. 1962. Also
inguinal hernia of right side. Rheumatism in
right shoulder and arm. Stomach trouble. Some
heart trouble. Some trouble of head, effects my
eyesight, weak back. That said disabilities are not
due to vicious habits, etc. That he has not applied
for pension. That he has never been employed in
the military or naval service otherwise than stated
above. Appoints James Ireland as attorney. P. O.
address, 209 Wayne Ave., Dayton, Montgomery Co.,
