I, Fred John Rehak, aged 27 years, occupation Ranch employee, do declare an oath that my personal description is: Color - white, Complexion - dark, Height - 5 ft. 8 1/2 ins., Weight - 175 pounds, Color of hair - dark brown, Color of eyes - brown.

I was born in Zlin, Moravia, Austria on 6th day of Nov., 1885. I now reside on a ranch 5 miles north of Matagorda, Texas. I emigrated to the United States of America from Hamburg, Germany on vessel Marzlien; my last foreign residence was Ong Hardisch, Austria; I am married the name of my wife is Clara Belle Rehak; she was born in Texas and now resides at Matagorda, Texas.

It is my bona fide intention to renounce forever all allegiance and fidelity to any foreign prince, potentate, state or sovereignty and particularly to the Republic of Austria. I arrived at the port of New York, in the State of New York on or about the 1st day of May, 1893.

I am not an anarchist. I am not a polygamist nor a believer in the practice of polygamy; and it is my intention in good faith to become a citizen of the United States of America and permanently reside therein.

I was born in Ardfert, Ireland on the 24th day of November, 1896; I now reside at 1111 Pierce Ave., Houston, Texas.

I emigrated to the United States of America from Liverpool, England on the vessel Baltic; My last foreign residence was Ardfert, Ireland; I am not married.

It is my bona fide intention to renounce forever all allegiance and fidelity to any foreign prince, potentate, state or sovereignty and particularly to George V, King of Great Britain and Ireland of whom I am now subject. I arrived at the port of New York, in the State of New York on or about the 9th day of Nov., 1919.

I emigrated to the United States of America from Poland on the vessel Olympic; my last foreign residence was Karenzyno, Poland. I am married; the name of my wife is Bertha; she was born at Austria, Hungary, and now resides in Wharton.

It is my bona fide intention to renounce forever all allegiance and fidelity to any foreign prince, potentate, state or sovereignty and particularly to the Austria Hungarian Empire of whom I am now a subject. I arrived at the port of New York, in the State of New York on or about the 25th day of Nov., 1920.

I emigrated to the United States of America from Poland on the vessel Berengaria; my last foreign residence was Knyshin, Poland. I am married; the name of my wife is Pearl Alexander; she was born in Khyshin, Poland and now resides at Bay City, Texas.

It is my bona fide intention to renounce forever all allegiance and fidelity to any foreign prince, potentate, state or sovereignty and particularly to the Republic of Poland of whom I am now a subject. I arrived at the port of New York, in the State of New York on or about the 18th day of Oct., 1921.

I emigrated to the United States of America from Austria, Hungaria on the vessel (do not remember); my last foreign residence was Austria, Hungaria; I am married; the name of my wife is Bertha; she was born July 30, 1888 and now resides at Iago, Wharton Co., Texas.

It is my bona fide intention to renounce forever all allegiance and fidelity to any foreign prince, potentate, state or sovereignty and particularly to the King of Hungaria, of whom I am now a subject. I arrived at the port of New York, in the State of New York on or about the 24th day of Sept, 1899.

I arrived at the port of Galveston in the state of Texas on or about the 9th day of Oct., 1908.

I emigrated to the United States of America from Cherbourg, France; I am married; the name of my husband is Hershel Alexander; he was born Mar. 15, 1881, Poland and now resides at Bay City, Texas; married Aug. 27, 1913.

It is my bona fide intention to renounce forever all allegiance and fidelity to any foreign prince, potentate, state or sovereignty and particularly to the Republic of Poland of whom I am now a subject. I arrived at the port of New York, in the state of State of New York on or about the 17th day of Oct., 1921.

Masaryk of Bohemia of whom I am now a subject. I arrived at the port of Galveston in the state of Texas on or about the 20th day of Nov., 1906.
