To The Town Clerk of the Town ofStockbridge Vermont

WE CERTIFY that we have carefully examined each of the several persons, residents of the Town of Stockbridgewhose names are herinafter stated and have found each of said persons to be physically unable to perform military duty, and have given to each of said persons a certificate, in due form of law, signed by us, stating such disability; and that the age, residence, nature of the disability, cause of disability, and probably duration of disability of each person are correctly stated against the name of such person, according to our best knowledge, skill and judgment.
